Application Form: After reviewing our Enrollment Procedures and touring the campus of your choice, please print this form and mail the completed application to your chosen campus by February 1st of the School Year for which you'd like to enroll.
Immunization Form: Please print and complete this form after receiving notice that your child has been accepted into our program.
Physician's Report: Please submit this form to your child’s pediatrician after receiving notice that your child has been accepted into our program. The form must be completed and submitted by the physician.
Medication Release Form: Print and complete this form if your child requires medication during school hours. A new form must be completed each week if the medication is to be continued. This form must be completed for all prescription and over-the-counter medications.
Health Policy: These campus-specific policies give detailed information regarding Cedar Crest Academy’s health and safety guidelines.
Disaster Plan: These campus-specific plans give detailed information about Cedar Crest Academy’s response to a variety of potential disasters.
